Notes from the nurse....
1. Remember that the state of Ohio requires all incoming 7th graders to have one does or Tdap prior to entry. If your child has already received the immunization, please send an updated immunization record to the school as soon as possible.
2. Scoliosis screenings are performed during your child's P.E. class. If you wish for yor child to not be screened, you can find the "Scoliosis Screening Letter" in teh back of your child's agenda. Please complete the form and return it to the clinic. If you want your child to be screened, please do nothing.
